What is Home Health Care?

Home Health Care refers to a wide range of health care services that can be given in your home for an illness or injury. At 1st Meridian Home Health, our skilled nurses, physical therapists, occupational therapists, speech therapists, and social workers provide expert level care through individualized treatment plans.

Private insurance is accepted. The benefits provided by these plans vary greatly from policy to policy. We will clarify your coverage and work with you to maximize your benefits.

What If I Have Medicare?

If you are confined to your home and need intermittent skilled nursing and rehabilitation services, Medicare pays 100% of qualifying services provided in the home when ordered by a physician.

Available Services under Physician Supervision

SN Skilled Nursing (RN/LVN) provides nursing care treatment and instructions to the patient and family, coordinating with all others involved in the patient’s care. The services can include but are not limited to: dressing change and wound care, catheter care, supervision of medications, family/patient health teaching and counseling, and diabetic teaching.

PT Physical Therapy helps patients regain functional mobility, increase strength and improve range of motion. Therapists provide specialized treatment as prescribed by the physician. The services can include but are not limited to: 1) Therapeutic exercises related to specific loss of function. 2) Utilization of adaptive equipment and aid in rehabilitation.

OT Occupational Therapy increases the patient’s independence and ability to perform daily functions by teaching adaptive methods and use of special equipment.

ST Speech Therapy assists the patient regarding speech, swallowing, and cognitive issues that may have been impaired following illness or accident.

MSW Medical Social Worker Services help the patient and family deal with financial and social problems that interfere with the patient’s recuperation or stability.

HHA Home Health Aids assist the patient with activities of daily living such as bathing and personal hygiene, the preparing and serving of meals, light housekeeping, assisting with home exercise regimen, observing and reporting changes in patient’s condition and needs.
